I, POLICYHOLDER NAME, who have electronically consented, acknowledge that the information contained above is, to the best of my knowledge, true. I have read this Mechanical Breakdown Insurance Policy (“Policy”) in its entirety and I understand and agree to all of the provisions, terms, and conditions contained herein, including the exclusions, cancellation and transfer sections. I agree to purchase this Policy in exchange for payment of the Policy Purchase Price shown below. I understand that this Policy has been issued in accordance with the information contained on this Registration Page. THE TERMS AND CONDITIONS CONTAINED ON THIS REGISTRATION PAGE AND THE TERMS & CONDITIONS THAT FOLLOW ARE THE FULL AND COMPLETE POLICY BETWEEN THE PARTIES. NO ORAL REPRESENTATION OR STATEMENT SHOULD BE RELIED UPON BY THE CUSTOMER.

THE PURCHASE OF THIS MECHANICAL BREAKDOWN INSURANCE POLICY IS OPTIONAL AND WILL NOT BE A FACTOR IN THE PURCHASE PROCESS OR THE CREDIT APPROVAL PROCESS. NEITHER THE EXTENSION OF CREDIT, THE TERMS OF THE CREDIT, NOR THE TERMS OF THE RELATED COVERED VEHICLE SALE MAY BE CONDITIONED UPON THE PURCHASE OF THIS AGREEMENT. I have read this Vehicle Service Contract (“Agreement”) in its entirety and I understand and agree to all of the provisions, terms, and conditions contained herein, including the exclusions, cancellation and transfer sections. I agree to purchase this Agreement in exchange for payment of the Agreement Retail Price shown below. I understand that this Agreement has been issued in accordance with the information contained on this Registration Page. THERE IS AN AGGREGATE CLAIM LIMIT ASSOCIATED WITH THIS AGREEMENT. I understand that prior authorization from the Administrator is required on repairs covered by this Agreement. I further understand that any Mechanical Failure, loss, or damage that results from a Pre-Existing Condition is not covered by this Agreement.
